All filings must be made on Town of Yarmouth forms.
All applications must be submitted two weeks prior to the next scheduled Conservation
Commission meeting. Refer to Meeting Schedule.
Failure to follow the Administrative Checklist shall result in an Administrative Incomplete
Application and will not be scheduled for a Public Hearing.
Contact the Conservation Office if you need assistance.

On January 16, 1997, the Commission issued an Order of
Conditions for the above -referenced site, for the construction
of a walkway to existing stairs, stone wall, and construction of
patios.
A site visit was made in December of 2021. The wall was
constructed between 2 and 3 feet further from the resource area,
and is slightly shorter than proposed; the walkway near the wall
was shortened by 11' over what was proposed. The patios were
constructed substantially according to the plan, with the
exception that patio replaced a portion of the dwelling that was
removed (all work occurred within the existing footprint).
To the best of my knowledge and belief, the project was
completed in substantial compliance with the approved plan and
Order of Conditions. Photos are enclosed for reference.
We hereby request the issuance of a Certificate of Compliance.

B. Submittal Requirements
Requests for Certificates of Compliance should be directed to the issuing authority that issued the final
Order of Conditions (OOC). If the project received an OOC from the Conservation Commission, submit
this request to that Commission. If the project was issued a Superseding Order of Conditions or was the
subject of an Adjudicatory Hearing Final Decision, submit this request to the appropriate DEP Regional
