To the Conservation Commission,
Attached please find Certificate of Compliance application materials for a residential
house construction project issued in 1987.
From the available information I believe no work on the house construction / foundation
excavation work was started. However, it appears that the septic system was installed
with the remaining home construction activity followed later covered under a subsequent
project review, DEP Wetland File No. SE83- 1134. Information is sketchy however the
septic system, shown as existing on a plan for SE83-1134 appears to be consistent with
the approval. The limited project activity is eligible for a Certificate of Compliance.

B. Submittal Requirements
Requests for Certificates of Compliance should be directed to the issuing authority that issued the final
Order of Conditions (OOC). If the project received an OOC from the Conservation Commission, submit
this request to that Commission. If the project was issued a Superseding Order of Conditions or was the
subject of an Adjudicatory Hearing Final Decision, submit this request to the appropriate DEP Regional
