ARCHITECT’S SITE OBSERVATION REPORT

The purpose of the Site Observation Report is to record general observations of the work in progress at the time of the Architect’s visit.
WORK IN PROGRESS:
1) CFMF and interior partition framing
2) CMU at gymnasium
3) Exterior sheathing and parapet insulation installation
4) Window installation and stripping in of windows
5) Fire protection installation
6) AVB installation and prep for window installation
7) Roofing at Area B, and Blocking/prep for roofing at A
8) Underground Electrical; electrical rough-in
9) Underground plumbing
10) HVAC and plumbing overhead rough-in
11) Site work: soil compaction and prep for site retaining walls at upper athletic field
12) GPI is on site for inspection of subgrade prep. (see attached GPI report)
FIELD OBSERVATIONS:
PE visited the site on Thursday 01 06, to review ongoing progress.
1. At the time of observation, Colony was about 75% with sheathing Area C and was focusing on Areas B and A
ahead of Stanley’s roof installation. Rough carpentry for window RO’s in area C appears complete. Colony’s wall
and parapet framing and sheathing at Areas B and A high roof appears to be nearly complete, including the stair
B bulkhead.
2. Beacon Waterproofing’s AVB installation at Area D appears about 70% complete.
3. Interior partition framing is ongoing at the first and 2nd floors of Area C (see photo on page 3), as well as overhead
rough-in for plumbing and HVAC.
4. Colony is installing drywall in isolated locations on the interior of D. Electrical closets etc.
5. Ongoing MEPs 1st and 2nd floor D wing. Arden will be finished with overhead in D this week according to
Commodore.
6. Lambrian has commenced window installation in Area D and windows for Area C will be arriving on site next
week. Beacon is stripping in the AVB for windows behind Lambrian. (see photo on page 4).
7. At the time of observation Lambrian appeared to have finished framing the CW for the mock up.
8. At the time of observation Stanley was installing the roof insulation and membrane at the last C wing “link,” and at
the upper atrium roof in area B; blocking for the skylights is in place and parapets have been sheathed ahead of
parapet flashing.
9. The curb for the roof expansion joint at the future gym fire wall has been framed and sheathed and is ready for
Stanley (see photo on page 3). The stair bulkhead for stair B has been framed and fireproofed and is ready for
stairs to be installed.
10. Lighthouse finished the CMU elevator shaft at area B earlier in the week (see photo)

11. At the time of observation Lighthouse masonry had completed tenting the east and north walls of the gymnasium,
heaters had been set up, and the masons were laying the first course of CMU at the east wall perimeter wall of
the gymnasium (see photo).
12. Commodore and Silverback are prepping for the slab-on-grade pour at Area B, scheduled for January 18th. At
the time of observation the underslab vapor barrier had been put down at Area B and the carpentry for the pour
stop was about 90% complete. Steel mesh installation is just starting.
13. The pour for the concrete at the composite deck over the B wing elevator shaft, as well as the auditorium ‘lower
bowl’ is scheduled to occur at the same time as the B wing slab on grade.
14. Annese is still continuing to install underground electrical work at area A.
15. RBO has caught up with Annese and Harold Bros., and so has shifted back to site work; RBO is prepping
subgrade for retaining wall footings at lower field B, and GPI is back on site to monitor this work.
DISCREPANCIES, SPECIAL INSTRUCTIONS
10601. Knee walls at stair curtain wall. PE asked Colony to move the small knee walls under the curtain wall at the
academic wing stairs out by approximately 2” so that they will align with the inside-corner sheathing (i.e. the
sheathing at the wing walls), in order for the curtain wall to sit squarely on the knee wall. This occurs at each
of the two stairs in both C and D wings (see photo on page 4).
10602. Inside curve at auditorium east wall. Colony asked PE about the truncated arc at the inside corner (A wing
to B wing) and whether this can be squared off. PE asked colony to follow Lymo shop drawings.
10603. Vertical control joint vs horizontal control joint misalignment (at links). After being asked about this issue, PE
told Colony that lack of perfect alignment in plan is due to the sheathing location and is per contract
documents. Wall CJ will hit the floor CJ cover—just not in the exact center. (this item is being closed)
21603. (From the 12-16 site report) OF scuppers. Harold Bros. has installed the originally-specified ‘lamb’s tounge’
scuppers at the overflow outlets high on the building, in spite of revised (B14) detail, which had called for
changing to a square one. Update: still open
12401. (From 11-24 site report): Sunshade outrigger discrepancies: Update: It was observed that there are
actually four four windows (or window pairs) where the steel is within the rough opening. PE asked Colony /
CB to pad out the jambs until the steel is at least flush or slightly inside the edge of the R.O. This can be
done with minimal impact to window locations (1/4 or 3/8” impact, worst case), and Lymo will have to make
adjustments with the exact spacing of HPC panels.

FIELD REPORT
All issues were discussed with PMA, CB and SRC prior to leaving the site. Work Observed/Procedure
• Under-Slab Vapor Retarder Installation
• Air and Vapor Barrier Membrane (AVB) Installation
• Roofing Installation- Area Designated as Part C
Observations
• Stego Wrap Vapor Retarder is being installed at Part B with a minimum 6-inch laps, as required. Laps were taped utilizing Stego Tape, as required. (Figures 1 - 2)
• Under-slab penetrations are detailed utilizing Stego Tape, as required. (Figure 3)
• At Part D, north elevation, Gridlines D1-D15, GCP Perm-A-Barrier Aluminum AVB was installed by
Beacon Waterproofing and Restoration Inc (Beacon) prior to RJK’s observations. Primer/adhesive was applied to the DensGlass Sheathing. The membrane was installed in a water shedding fashion, with a minimum 2-inch side lap and 10-inch lap around outside corners. (Figures 9 - 10)
• At window rough openings, metal angle was installed at the sills fastened utilizing screws. The metal was then stripped in with a piece of AVB lapping from the rough opening sill onto the metal angles horizontal
and vertical legs. The strip of membrane was sealed with sealant along three edges. (Figures 11 - 13)
• Sika Sarnafil Sarnatherm CG polyisocyanurate insulation was installed loose laid over the vapor retarder in
two layers with offsets between rows and layers, as required. (Figure 14)
• Sika Sarnafil Sarnatherm High Density coverboard was installed over insulation with a field fastening pattern of 16-plates and fasteners per 4-foot by 8-foot board and a perimeter fastening pattern of 24- plates and fasteners per 4-foot by 8-foot board and corner fastening pattern, as required in the Sika assembly letter. (Figures 15 - 16)

• Sika Sarnacol-2170 VC Adhesive was roller applied to the coverboard and allowed to flash off prior to adhesion of the membrane, as required.
• The Sika Sarnafil G 410-80 EnergySmart PVC Roof Membrane was cut to size and allowed time to relax prior to adhesion. Sika Sarnacol-2170 VC Adhesive was roller applied to the back of the membrane and immediately the membrane was rolled onto the coverboard and rolled in utilizing a 100-pound roller, per the manufacturer’s requirements. (Figure 17)
• A test weld was taken utilizing the walk welder and scrap membrane. The first test resulted in a failure; a second was taken and resulted in a scrim tearing bond. (Figures 18 - 20)
• PVC membrane seams were welded utilizing the walk welder. (Figure 21)
Discussions
Below-Slab Vapor Barrier Installation
3.1 Voids/Fish Mouth in Laps - Part B- Folds in the membrane near laps has created fish mouths and voids in the vapor retarder under the tape. The areas should be cleaned, dried, and new tape applied. (Figure 8)
3.2 Voids in Penetration Detailing - Part B - Voids in the applied tape must be filled with new tape or Stego Mastic as required. (Figures 4 - 5)
3.3 Tape Unadhered to Column Footing - Part B - Stego Tape does not adhere to concrete. Detail column footings into the vapor retarder utilizing Stego Mastic or other manufacturer approved product (Figures 6 -
7)
Roofing Installation
3.4 Wet Polyisocyanurate Insulation and Coverboard Installed - Part C - Insulation and coverboard has become wet in storage and SRC is installing the insulation and coverboard into the roofing system prior to allowing the insulation to fully dry and checking for delamination. Multiple moisture tests were performed to confirm elevated moisture content of the glass facers, as well as to show standing moisture on the insulation and
coverboard. A Delmhorst BD-2100 Moisture Meter was utilized to check the moisture content resulting in readings up to “97.8” on the reference scale (0-100 range). Insulation set in the first layer exposed at the edge of the area of work was found to be wet hours of being left out. Juan (SRC) explained they were waiting to glue membrane until the coverboard dried, although the weather did not promote drying. (Figures 22 - 30)
3.5 Incomplete Night Tie-In - Part C - The northeast corner of the night tie-in was left open, exposing the polyisocyanurate insulation and coverboard. Moisture readings of the facers, utilizing a Delmhorst BD-2100 Moisture Meter, showed readings up to “33.9” showing some moisture gain, likely from humidity. The night
tie-in should be made watertight to prevent moisture from entering the roofing system in case of rain, snow, or heavy fog. (Figures 33-35)

Work Observed/Procedure

Observations
• Stego Wrap Vapor Retarder is being installed on prepared grade with a minimum 6-inch lap, as required.
Laps were taped utilizing Stego Tape, as required. (Figure 1)
• Under-slab penetrations are detailed utilizing Stego Tape, as required. (Figure 2)
• Stego Mastic was applied to the concrete footing and metal column and the Stego Wrap lapped into the
mastic. Stego tape was utilized at multiple locations around the perimeter of the column, although it is only
needed at the seams in this detail. (Figure 3)
• At Part C, south elevation, GCP Perm-A-Barrier Aluminum AVB was installed by Beacon Waterproofing
and Restoration Inc. (Beacon) prior to RJK’s observations. Primer/adhesive was applied to the DensGlass
Sheathing. The membrane was installed in a water shedding fashion, with a minimum 2-inch side lap and
10-inch lap around outside corners. (Figure 4)
• At window rough openings, a strip of AVB was installed extending past the jamb and onto the exterior
sheathing. The horizontal field sheets were run into the jambs in water shedding fashion. A head strip was
then installed lapping over the jamb strips, as required. (Figure 5)

• Gaps in the insulation exceeding the maximum 1/4-inch requirement are filled utilizing new
polyisocyanurate insulation. (Figures 12, 13)
• Sika Sarnafil Sarnatherm High Density cover board was installed over insulation at Part C, with a field
fastening pattern of 16-plates and fasteners per 4-foot by 8-foot board, a perimeter fastening pattern of 24
plates and fasteners per 4-foot by 8-foot board, and corner fastening pattern, as required in the Sika assembly
letter. (Figure 14)
• Georgia-Pacific DensDeck Prime coverboard and USG Securock was installed over insulation at Part B, with
a field fastening pattern of 16-plates and fasteners per 4-foot by 8-foot board, a perimeter fastening pattern of
24 plates and fasteners per 4-foot by 8-foot board, and corner fastening pattern, as required in the Sika
assembly letter. (Figures 15, 16)
• Prefabricated outside corners are heat welded at outside corner transitions of the thermoplastic polyolefin
membrane baseflashing. (Figure 17)

Below-Slab Vapor Barrier Installation
4.1 Unadhered/Missing Stego Tape - Part B - Tape was not fully adhered and some laps were missing tape.
(Figure 18)
Window Installation
4.2 Knife Plates in Rough Opening - Parts D & C - Knife plates run into the rough openings, impeding
installation of the window and interior sealant joints. (Figures 19 - 21)
4.3 Interior Air Barrier Seal at Windows - Part D - Shims utilized to offset the window from the rough opening
are set close to the interior face of the window frame, creating a gap inadequate for proper backer rod and
sealant joint detailing. If the shims cannot be moved deeper into the frame, as to not interfere with the
backer rod and sealant, then backer rod should be cut between shims and bond breaker tape applied to shims
prior to application of sealant which is to be dry tooled only. (Figure 22)
4.4 Window Fastening - Part D - Screws fastened through the interior fin penetrate the AVB without sealant.
Although the membrane passes ASTM D 1970, it is recommended as best practice to add sealant at the
screw location for wet setting each screw, specifically at the sill. (Figure 23)
Roofing Installation
4.5 Wet Polyisocyanurate Insulation and Cover Board Is Installed - Part C & Part B - Insulation and cover board
has become wet in storage. Multiple boards were set aside that were wetted, although multiple boards are
observed installed with staining. Due to the multiple areas with questionable moisture levels, testing at the
end of installation of the roofing system is recommended to check for unacceptable levels of moisture within
the system. (Figures 24, 25)

4.6 Pitch Pocket Detail - Part C - A Sarnastack prefabricated pipe flashing is utilized as the pitch pocket to
contain the Sarnafiller pourable sealant. Sarnafil details show Sika Sarnafil Sarnaclad PVC-coated sheet
metal pitch pockets, a minimum of 2-inches from the penetrations, as the only way to detail this condition.
The condition should be reviewed with the manufacturer for approval, or the detail must be installed per the
manufacturer’s published details. (Figures 26, 27)
4.7 Uncut Clamping Rings - Part C - Multiple clamping rings installed at pipe penetrations were left uncut. This
condition can allow the long metal ring to strike the membrane in windy conditions. (Figure 28)
4.8 No Clamping Ring - Part C - A penetration did not have a clamping ring installed. Since the sealant has
dried, new sealant must be applied prior to installation of a clamping ring. (Figure 29)

This Report is in reference to the proposed Retaining Wall installation that is located Due South of Athletic Field # 2.
Construction Notes: The earthwork construction procedures, (foundation preparation) for this installation were
performed in accordance with the design documents submitted by COWEESET ENGINEERING, of West Bridgewater,
Massachusetts – Segmental Retaining Wall # 1, Orientation Plan dated: June 7, 2021. References to Wall Location,
orientation, stationing and other ancillary construction specifications that are not directly linked to Foundation
Preparation are also included in this submittal. The aforementioned documents are not included in this Earthwork
Monitoring Daily Field Report.
Excavate to design subgrade elevation using best practices
Place a 12” thick layer of Compacted ¾” Crushed Stone upon subgrade
Retaining Wall Blocks may now be placed upon the Crushed Stone surface
In‐Situ Native Sand Preparation Detail :
The soil matrix encountered at subgrade elevation largely consists of uniform sands and fine sands with scant traces of
non‐plastic silt. Therefore, the proper in‐situ compaction of this material is largely dependent on the relative moisture
content of the soil. Since the encountered materials are of native, (natural) deposition the water content can vary from
location to location. Each situation must be evaluated to determine that the proper soil compaction techniques are
utilized vis‐à‐vis the moisture content of the soil. In other words, it may be necessary to moisten some areas with
supplied water in order to certify proper compaction of the native sand subgrade in these circumstances.
