WORK IN PROGRESS: 1) CFMF and interior partition framing 2) Drywall 3) CMU at gymnasium 4) Exterior sheathing and parapet insulation installation 5) Window installation and stripping in of windows 6) Fire protection installation 7) AVB installation and prep for window installation 8) Roofing at Area B, and Blocking/prep for roofing at A 9) Electrical rough-in 10) HVAC and plumbing overhead rough-in 11) Rustic is in the process of mobilizing for fire protection installation FIELD OBSERVATIONS: PE visited the site on Thursday 01 06, to review ongoing progress. 1. At the time of observation, Colony had mostly finished sheathing Area C and the ‘links’/ connectors, as well as the upper walls in Areas B and A (see photos). Colony is in the process of sheathing the stair bulkhead on the roof of B and the south wall of Area B on level 2. 2. AVB installation at Area D (by Beacon) appears to be about 90% complete. 3. Interior partition framing is ongoing at the first and 2nd floors of Area C. 4. HVAC and plumbing overhead roughing is ongoing in 1st and 2nd floor C wing. 5. Colony is installing drywall in isolated locations on the interior of D. Electrical closets etc. 6. Lambrian is continuing window installation in Area D and Beacon is continuing to strip in the windows; Lambrian has finished framing and glazing the curtain wall for the exterior wall mock up. 7. At the time of observation Stanley was still installing the roof membrane at the at the upper atrium roof in area B (see photo on p. 4); and was also detailing the boots for the guard rail stanchions at Area C. 8. The plywood parapets at the upper auditorium have been framed and sheathed in plywood, and are ready for membrane. 9. United has installed the upper flight of stairs between level 2 and the roof for stair B. 10. At the time of observation Lighthouse masonry was continuing to lay ground faced block at the perimeter wall of the gymnasium (see photos on p. 4). The masonry work appears to be consistent with the contract drawings. 11. Commodore and Silverback are prepping for the slab-on-grade pour at Area B, scheduled for January 18th. The area for the SOG pour has been tented for heat and steel mesh for the SOG pour is in place (see photo). Blankets have been installed at Area A to prevent freezing. 12. Stellar was back on site earlier this week; they have installed the metal deck on top of B wing elevator shaft, as well as the metal deck at the auditorium “lower bowl” area (see photo on p. 3) 13. Annese is waiting for underground electrical work inspection at area A; crushed stone installation for A is pending, and RBO was not on site today. REPORT BY: ABH Page 2 of 4 DISCREPANCIES, SPECIAL INSTRUCTIONS 11301. Conflict between light shelf framing and radiant panels. PE and Girard had a discussion with CB regarding conflict between steel ‘outriggers’ for the ‘lightshelf’ framing and radiant panels. The direction is to ‘bump up’ the outriggers up so that the radiant panels can fit below them at the design elevation. 11302. Guard rail welds at D & C stairs - United still needs to grind welds for guards at academic stairs (see photo). 10601. (From 01-06 site report): Knee walls at stair curtain wall. PE asked Colony to move the small knee walls under the curtain wall at the academic wing stairs out by approximately 2” so that they will align with the inside-corner sheathing (i.e. the sheathing at the wing walls), in order for the curtain wall will sit squarely on the knee wall. This occurs each of the two stairs in both C and D wings. Update: still open 10602. (From 01-06 site report): Inside curve at auditorium east wall. Colony asked PE about the truncated arc at the inside corner (A wing to B wing) and whether this can be squared off. PE asked colony to follow Lymo shop drawings. Update: still open (the ACM is radiused, so the curve needs to be framed out and sheathed). 21603. (From the 12-16 site report) OF scuppers. Harold bros has installed the originally-specified ‘lamb’s tounge’ scuppers at the overflow outlets high on the building, in spite of revised (B14) detail, which called for changing to a square one. Update: still open 12401. (From 11-24 site report): Sunshade outrigger discrepancies: It was observed that there are actually four four windows (or window pairs) where the steel is slightly within the rough opening. PE asked Colony/CB to pad out the jambs until the steel is flush or slightly behind the edge of the R.O. This can be done with minimal impact to window locations (1/4 to 3/8” impact, worst case). Update: this was done. This Item is being closed. Engineers, LLC 10 Waterchase Drive Rocky Hill, CT 06067 860.563.3820 www.girardco.com Field Observation Report - 09 January 18, 2022 Attn: Daniel Colli, AIA Perkins Eastman, DPC Boston, MA Re:Site Visit – January 13, 2022 Dennis Yarmouth Intermediate Middle School Yarmouth, MA On Thursday, January 13, 2022, our office visited the job site to observe the construction progress for the Dennis Yarmouth Intermediate Middle School The Construction trades working this day included concrete, masonry, misc-steel, cold-form, plumbing, electrical, and site contractors. During the visit, I spoke with representatives from Commodore Builders, owner representatives from PMA, and walked the site with Andy Hazelton. Building earthwork continues with stone placed in the cafeteria and kitchen area. The stone is protected with heavy tarps. The under-slab stone layer is not complete in the auditorium area. The cold-form metal framing in all areas is in progress, and windows are being installed in area D. The steel deck is being installed at the auditorium sloped floor. The area B slab on grade is being prepared for placement. Vapor-retarder is installed, and the welded wire fabric is being placed on concrete bricks. Temporary tarps are being hung around the perimeter and over the atrium opening. Masonry has started in the Gym along the East exterior wall. Elevator shafts are complete with temporary openings provided for the installation of the elevator equipment. Winter protection methods are employed with temporary enclosures. The following questions from site superintendent Paul Genereux were answered. There is a small section of floor in the auditorium that is flat for the control desks on the Southeast corner of the sloped floor. Does the edge of the slab need special reinforcement? No, the slab can be formed to create the small step at this area without additional reinforcement. Dowels are coming out of the tie wall between columns A28/M.5 and b21/M.5 but no detail on how they dowel into the slab. Do these dowels need to be turned out 90 degrees from the wall and into the slab? No, The dowels can be left in line with the wall. Concrete housekeeping pads are detailed at 6” high. Can these be installed at 3 ½” high? Yes but, verify the detailing of any elevations for pre-fabricated piping or conduits to the pad installed equipment. While walking the site with Andy, we reviewed the interior radiant panel detailing with Ryan from Commodore builders. A single section of the misc-metals frame has been installed to verify the detailing with the mechanical radiant panels. We reviewed the detailing with Ryan, and there will need to be some additional coordination work between the trades before the misc-metals framing can continue. Field Observation Report - 09 Dennis Yarmouth Intermediate Middle School Yarmouth, MA Page: 2 of 3 Girard & Co. Engineers, LLC January 18, 2022 In general, the work observed appears in conformance with standard industry practice. Other than the items noted above, there were no apparent deficiencies noted. However, there are open items on the inspection log for deficiencies or incomplete items, and some of these items require our review. We have reviewed the open items and recommend the following. The inspection of the cold-formed metal framing on 12/21/21 has noted incomplete items and non- compliance with screw connections. The deligated design engineer for the cold-formed metal framing will need to review the non-compliance items. As mentioned in our last field report, concrete test # 17282 indicates a low break for the concrete placed in the metal pan stairs in area D. We reviewed the stairs during our visit. There is an area in the West stair landing that has surface spalling. It is unclear what caused this damage, but it’s only in one small area and is similar to freeze-thaw damage. The rest of the stair landings and treads, along with the East stair, are all performing well under the heavy construction traffic. The 56-day break was still under the design compressive strength, and a final cylinder is being held for a 90-day break. We would recommend a response from the installing contractor and concrete supplier to verify the concrete proportions and provide a recommended repair for the damaged concrete.
