Subject:RE: Certificate of Compliance / Dredging permit extension
Hi Jeff
1. For SE83-2041 you will need an as-built plan and statement from engineer on compliance with the order/approved
plan. Here is a link to the filing forms https://www.yarmouth.ma.us/696/Filing-Forms
2. For the dredging permit, the Governors Order No. 42 tolled permits during COVID and extended them for the duration
of the declared emergency so your permit is automatically extended by 462 days. After that period you will likely need
to file a new Notice of Intent. You can request a further extension but the Commission will likely require a new NOI due
to the time lapsed and the need to assess the project for any impacts etc.

Hi Kelly,
I hope that this note finds you safe and well.
I would like to review a few issues with you-
1. I would like to apply for a Certificate of Compliance. All of the plantings are compete per the order of conditions. Is
there a form needed to request this?
2. I will be doing maintenance dredging once again in the beginning of June. It seems that the beach area in front of my
home is a net deposit area for sand - especially in the winter. My dredging permit expires on 6/22/22. Given the sand
build up issues, I need to renew the dredging permit. Please advise on how best to do this. Can I simply apply for an
extension of what is on file today? I know I need to apply to the DEP. I am assuming that I need to get a permit from the
town as well. I just want to get ahead of it now given how busy everyone is.

I would be happy to discuss with you if it is easier or more time efficient. Please advise when you have a moment.
Thank you.
Sincerely,
Jeff Chizmas
