I, Darren M. Meyer, Registered Sanitarian, duly licensed as such in the
Commonwealth of Massachusetts, do hereby certify that I have visually inspected
the constructed subsurface sewage disposal system, and further certify that the
system, as constructed, generally conforms within acceptable tolerance to the
regulations,as varied, set forth in 310 CMR 15.000 and the Town of Yarmouth Board
of Health Regulations.
This letter certifies the location and type of system installed, it's compliance with
applicable regulation. This certification should not be construed as a guarantee the
system will function as installed.
