On behalf of the applicant, we hereby submit this application for the construction of the system
at the above referenced address. The applicant proposes to install 1500-gallon septic tank, a
distribution box, and 2-65' long x 2' wide x 1.3' deep leaching trenches. Enclosed please find the
following:
1. 4 sets of the Site Plan, dated October 18, 2021.
2. Application for Disposal System Construction Permit.
3. Check for $110 Application Fee payable to the Town of Yarmouth.
4. Cape Cod Commission high-groundwater adjustment worksheet.
