On June 23,2020, 1 inspected the construction of the septic system at the above referenced
site.The system has been installed in accordance with the plan prepared by this office.
dated 5-28-20.
The existing septic tank was found to be in good working condition at the time of
construction.
I certify that the system as constructed,generally conforms within acceptable tolerance to
the regulations,as varied,set forth in 310 CMR 15.000 and the Town of Yarmouth Board
of Health Regulations.
