You are able to submit the Request for Certificate of Compliance yourself however you will need a statement from a
certified professional – engineer or surveyor, certifying that the work was completed according to the approved plans
and Order.
Once you have that you are able to submit the request with the $75 filing fee. Here is a link to the filing

I have been trying to reach Lynne Hamlyn for the last month with no luck, Her phone is disconnected and nor
erspsonose on email.
Since we completed what was required of us I believe the filing is what is left to do.
Can I complete the Requests for Certificate of Compliance? If so , would you direct me to that form?
