In accordance with your authorization, floor flatness tests were performed on the Slab on metal deck, Ground Floor
Part B, Line M.5-Q, Row a18-Pass a30, for the concrete placement on February 9, 2022. An Allen Face and Company
D-Meter Model 1610, S/N 0324 was used. Measurements were performed in general accordance with ASTM E1155
– 2008 and Project Manual, Section 033000 Cast-in-Place Concrete. Summary of the test performed is outlined below
