Dear Ms. Langler,
As per your request and in accordance with 310 CMR 15.021(3) of the State
Environmental Code-Title 5, an as-built review of the septic system at the subject site
has been conducted. I hereby certify that the Sanitary Subsurface Disposal System
components have been installed in substantial compliance with the design plan by
Engineering Works, Inc., dated 11/12/20. The septic system consists of a new
1500/500 gallon septic tank/pump chamber, new distribution box and a new 15' wide x
30' long leach field. The inspection was performed prior to backfill. It is the
responsibility of the contractor to provide the final grading as proposed on the design
plan.
