All applications must be submitted two weeks prior to the next scheduled Conservation
Commission meeting. Refer to Meeting Schedule.
Failure to follow the Administrative Checklist shall result in an Administrative Incomplete
Application and will not be scheduled for a Public Hearing.
Contact the Conservation Office if you need assistance.

On March 20, 2009 the Commission issued an Order of Conditions
for the above -referenced site, for landscape work (trimming,
pruning of branches, management of invasives) on this vacant
lot. Subsequently, Wilkinson Ecological Design requested an
Amended Order to encompass Phragmites treatment and removal.
A site visit and location survey were performed in March of
2022. The following deviation from the record plan and Order of
Conditions is as follows:
o The proposed blueberry/viburnum plantings were not in
evidence
The lawn/shrub border has remained essentially the same since
2009, based on the survey. The buffer of shrubs (primarily
viburnum) has evidence of past trimming (not recent), look
healthy and robust, and provide a dense buffer to the wetland.
No invasive vines were in evidence.
With the exception of the listed deviation, and to the best of
my knowledge and belief, the project was completed in
substantial compliance with the approved plan and Order of
Conditions.
We hereby request the issuance of a Certificate of Compliance.

B. Submittal Requirements
Requests for Certificates of Compliance should be directed to the issuing authority that issued the final
Order of Conditions (OOC). If the project received an OOC from the Conservation Commission, submit
this request to that Commission. If the project was issued a Superseding Order of Conditions or was the
subject of an Adjudicatory Hearing Final Decision, submit this request to the appropriate DEP Regional
