All filings must be made on Town of Yarmouth forms found at www.ya1mouth.ma.us/325/Conservation. The
filing deadline is two weeks prior to the next scheduled Conservation Commission meeting. No new
information may be accepted into the Commissioners' packets within the week before a meeting.
Failure to follow this Checklist shall result in an Administrative Incomplete Application and will not be
advertised for a Public Hearing. Refer to the meeting and fee schedule. Contact the Conservation Office if you
need assistance.

map and parcel numbers. Assessor's maps can be found at www.varmouth.ma.us/70/Assessor
1 original and 7 photocopies of the plan/sketch, folded separately, right side out with title visible. All plans
shall reference NAVD1988 unless otherwise noted. Landscape plans shall be detailed to show proposed
and existing conditions, native species, size and spacing.

REQUEST FOR DETERMINATION OF APPLICABILITY
717 Willow Street, South Yarmouth, MA
Project Narrative
The applicant is pleased to submit a Request for Determination of Applicability
application for an addition and septic system upgrade. The 17,436 square foot property
is surrounded by existing dwellings in a coastal area of South Yarmouth. This single-
family home is more than 1000 feet from the Bass River and any Priority or Estimated
Habitat of endangered species. Therefore, no work will take place in either NHESP or
coastal resource areas.
The resource area affecting this the site is Land Subject to Coastal Storm Flowage
(LSCSF) AE EI. 11. All other resource areas are located greater than 100' from the
property.
Site improvements for this project are an addition of living space in an area now occupied
by a deck. Renovations to portions of the existing dwelling. And a new septic system to
include a new septic tank and leaching field.
The site was surveyed by Down Cape Engineering Inc. and the topography is based on
the current NAVD '88 flood zone datum. No increase in the number of bedrooms is
proposed. No fill is expected to be required for this project.
