Due to COVID permits valid during the state of emergency were extended per this guidance

By my calculation, this extends your Order to 1/1/2024 (permit is tolled 462 which is the length of the state of
emergency)
When you want to request an extension, you do so in writing with a $75 check. The Order will extend from the date of
expiration. I believe the NHESP determination is only valid for 5 years so the Commission may only extend the permit
until further NHESP review is required. This would still get you through until Sept 2024.

As our OofC needs to be renewed at least 30 days prior to three years after the issuance date of (9-26-19), and I
understand we can do that for three year increments:
(a) is there a form for this renewal (I could find one), and (b) if we renew it in say April, does it extend it from that date
or three years from the original expiration date(9-26-22)?
