the following information etc. to sign off on the Transmittal:
- The floor plan of the existing dwelling.
- A description of the foundation for the existing barn if any.
- A title 5 application signed by a licensed installer with the appropriate fee.

Good afternoon Steve,
We have now received ZBA's approval to move forward with this project. Attached are pictures of detailed
floor plan of the existing dwelling. The pictures are from the plans available on record at the health department
from when the home was restored.
The foundation of the existing barn is 2 rows of 8in blocks.
The title 5 application by a licensed installer was submitted today with the Health department.
Please advise if you need anything else from our side in order to sign off.
