Dear Todd:
I have reviewed the septic system site plans for the above addresses and have the
following comments:
435 North Main Street:
1. The septic system profile lists " 3 flowdiffusors" instead of the proposed "5
flowdiffusors" as specified in the design calculations.
2. In accordance with the local septic regulations, a registered land surveyor stamp
must be provided. A plan of reference that provides the location of the dwelling
on the lot along with a land surveyor stamp may be substituted if a copy of that
plan is submitted with the sewage plan. Please be advised that the Yarmouth GIS
system is not an acceptable substitute.
3. The floor plan provided does not correlate with the Assessor records of four(4)
bedrooms total. Prior to Health Department approval of a fifth bedroom located
in the basement, prior approval of the additional bedroom by the Building
Department must be provided. The leaching facility may be over designed,
however, the design calculations must list a four(4)bedroom design.
101 Captain Small Road:
1. The subject property appears to fall within Zone C of Index Well AIW-230.
Please correct the groundwater adjustment and subsequently, the elevations of the
proposed leach facility to maintain a minimum of 4' above adjusted groundwater.
2. As noted above, please provide a registered land surveyor stamp, or a copy of an
appropriate reference plan.
Please submit four(4)revised copies of both plans prior to final approval. If there are
