INSTALLATION GENERAL NOTES
1. ALL MATERIALS AND CONSTRUCTION METHODS SHALL CONFORM TO THE PROVISIONS OF THE
COMMONWEALTH OF MASSACHUSETTS ENVIRONMENTAL CODE TITLE 5.
2. EXCEPT AS OTHERWISE NOTED, ALL PROPOSED SEPTIC SYSTEM PIPING SHALL BE 4" 0 SCH40 PVC
SET TO THE LINE AND INVERT ELEVATIONS SHOWN. THE MINIMUM PITCH OF PIPES CARRYING SEWAGE
OR SEPTIC TANK EFFLUENT SHALL BE 1/8TH INCH PER FOOT IF NOT OTHERWISE NOTED.
3. PRIOR TO CONSTRUCTION OF THE SEPTIC SYSTEM DEPICTED ON THIS PLAN, THE SEPTIC SYSTEM
INSTALLER (INSTALLER) SHALL OBTAIN A DISPOSAL WORKS CONSTRUCTION PERMIT FORM THE TOWN
OF YARMOUTH HEALTH DEPARTMENT. THE SEPTIC SYSTEM INSTALLER SHALL BE RESPONISBLE FOR
OBTAINING A TRENCH PERMIT FROM THE MUNICIPALITY IN WHICH THE WORK IS PERFORMED
4. THE LOCATIONS OF UNDERGROUND UTILITIES SHOWN ON THIS PLAN ARE APPROXIMATE AND MAY
NOT BE COMPLETE. AT LEAST 72 HOURS PRIOR TO ANY EXCAVATION FOR THIS PROJECT WORK, THE
INSTALLER SHALL MAKE THE REQUIRED NOTIFICATION TO DIG SAFE (811), AND THE YARMOUTH WATER
DEPARTMENT (508) 771-7921 FOR VERIFICATION OF LOCATIONS.
5. CONSTRUCTION OF THE SEPTIC SYSTEM COMPONENTS SHOWN ON THIS PLAN IS SUBJECT TO THE
INSPECTION OF THE TOWN OF YARMOUTH HEALTH AGENT. NO PART OF THE SEPTIC SYSTEM SHALL
BE BACKFILLED OR MADE INACCESSIBLE UNTIL INSPECTED AND APPROVED BY THE HEALTH AGENT
AND INSPECTED AN CERTIFIED COMPLETE ACCORDING TO THE APPROVED PLAN BY DAVID C. THULIN,
PE PLS. THE INSTALLER SHALL SCHEDULE INSPECTIONS AS REQUIRED,
6. REMOVE ALL UNSUITABLE SOIL, A, B & C1 HORIZONS FROM WITHIN FIVE FEET LATERALLY AND
UNDER THE PROPOSED SOIL ABSORPTION SYSTEM AND REPLACE WITH CLEAN SAND MEETING THE
REQUIREMENTS OF 310CMR 15.255.
7. WATER SUPPLY FOR THIS LOT IS PUBLIC WATER CONNECTED AT THE EXISTING WATER MAIN IN THE
APPROXIMATE LOCATION SHOWN. THE PROPOSED SEPTIC SYSTEM SOIL ABSORPTION SYSTEM IS NOT
TO BE LOCATED WITHIN 150' OF AN EXISTING PUBLIC OR PRIVATE WATER SUPPLY.
8. All SYSTEM COMPONENTS (INCLUDING EXISTING TO REMAIN) SHALL BE FITTED WITH CONCRETE
RISERS AND COVERS IN ACCORDANCE WITH TITLE 5 ACCESSIBLE WITHIN SIX INCHES OF FINISH GRADE.
THE FOLLOWING VARIANCES AS MAY BE GRANTED BY LOCAL UPGRADE APPROVAL ARE NECESSARY TO
CONSTRUCT THE SEPTIC SYSTEM AS SHOWN IN ACCORDANCE WITH TITLE 5 SECTION 15.405
