you meet the requirements of section 15.255(2) so that it is clear to the installer.
Also could you expand the inspections required by you to include certifying soils and final grading.

Hi David,
My name is Anna Wimmer, and I am helping Steve with Title V review. Steve is allowing me to use his email while mine
is being set up.
I've reviewed your plan for the proposed system at the above address, and the following changes will need to be made
before we can approve it:
1. A note should be added to the plan stating that the designer/engineer is to inspect and certify the system once
installed;
2. The septic tank needs to be watertight and factory waterproofed (this should be called out on the plan);
3. A second weep hole needs to be added above the check valve;
4. The flow line in the pump chamber needs to be 2" minimum and called out as such;
5. A note should be added stating the requirement for an electrical permit for the pump chamber;
6. A note should be added stating that the leaching field will be staked out by the designer prior to construction.
The leaching field is too far away from the house for swing ties to be accurate;
7. More existing elevation data is needed in the area of the leaching field; and
8. Due to the proximity of the leaching field to two of the property lines, a profile is required to show five feet of
flat grade between the bottom of the slope and the property line OR a swale that will keep storm water from
leaving the property.
Please submit the required number of revised plans to the health department for review.
