All applications must be submitted two weeks prior to the next scheduled Conservation
Commission meeting. Refer to Meeting Schedule.
Failure to follow the Administrative Checklist shall result in an Administrative Incomplete
Application and will not be scheduled for a Public Hearing.
Contact the Conservation Office if you need assistance.

On February 24, 2016, the Commission issued an Order of Conditions for the above -
referenced site, for the construction of a single-family dwelling at this property.
A site visit was made in February of 2022. The proposed dwelling, porches, decks, and
septic system were constructed per the approved Site Plan dated 12/28/2015, A small
patio area was constructed under the rear deck and a small fire pit exists in the back
yard. A retaining wall was constructed in lieu of the proposed grading around the septic
system and driveway areas. Gravel was placed around the retaining wall and house
which should provide the same level of runoff infiltration as the proposed natural areas
and swales shown on the approved plan.
Notwithstanding the aforementioned changes, the proposed work was constructed in
substantial compliance with the approved plan and Order of Conditions. An as -built site
pIan has been prepared and is attached hereto.
We respectfully request the issuance of a Certificate of Compliance.

B. Submittal Requirements
Requests for Certificates of Compliance should be directed to the issuing authority that issued the final
Order of Conditions (OOC). If the project received an OOC from the Conservation Commission, submit
this request to that Commission. If the project was issued a Superseding Order of Conditions or was the
subject of an Adjudicatory Hearing Final Decision, submit this request to the appropriate DEP Regional
