Dear Mr. Murphy,
As per your request and in accordance with 310 CMR 15.021(3) of the State
Environmental Code-Title 5, an as-built review of the septic system at the subject site
has been conducted. I hereby certify that the Sanitary Subsurface Disposal System has
been installed in substantial compliance with the design plan by Engineering Works,
Inc., dated 11/10/21, revised 2/25/22. The septic system consists of an existing 1000
gallon septic tank which was observed to be structurally sound, a new 1000 gallon
monolithic pump chamber, a new distribution box and a 13' x 35' leach field, as shown
on the plan.
