Dear Mr. Murphy,
As per your request and in accordance with 310 CMR 15.021(3) of the State
Environmental Code-Title 5, an as-built review of the septic system at the subject site
has been conducted. I hereby certify that the Sanitary Subsurface Disposal System has
been installed in substantial compliance with the design plan by Engineering Works,
Inc., dated 4/27/22 but having the following deviations from the plan. The D-box was
set on the opposite end of the field. Field was slightly rotated but met all required
building and property line setbacks. The pump chamber was turned 90 degrees.
All proposed elevations were satisfied.
