Dear Ms. Langler,
As per your request and in accordance with 310 CMR 15.021(3) of the State
Environmental Code-Title 5, an as-built review of the septic system at the subject site
has been conducted. I hereby certify that the Sanitary Subsurface Disposal System
components have been installed in substantial compliance with the design plan by
Engineering Works, Inc., dated 7/6/20. The septic system consists of an existing 1000
gallon septic tank, a new 1000 gallon pump chamber, new distribution box and a new
soil absorption system consisting of 5-precast LC-6 chambers surrounded with 3.5' of
stone. The inspection was performed prior to backfill. It is the responsibility of the
contractor to provide the final grading as proposed on the design plan.
