A visual inspection was done of the soils removal as well as a visual final inspection of the septic
installation at the above address. The system appears to be installed in accordance with the
approved plan dated January XX,2022 with the following notations:
At the time of inspection:
1. The system needs to be carefully backfilled and graded
2. The installer, Quirk Excavating is to supply the"As-built"cards
