Remarks: Enclosed is the recent 06M inspection form for the system at the above referenced location. The
average daily flow during this reporting period was 3,374 gallons per day. The 06M inspection form
indicate the system is operating properly. Test results indicate good treatment of the system. We will
adjust the system settings to help improve treatment of the system. Please do not hesitate to contact us
