Dear Mr. Murphy,
As per your request and in accordance with 310 CMR 15.021(3) of the State
Environmental Code-Title 5, an as-built review of the septic system at the subject site
has been conducted. I hereby certify that the Sanitary Subsurface Disposal System
has been installed in substantial compliance with the design plan by Engineering
Works, Inc. & JC Engineering, Inc., dated 5/3/22. The septic system consists of a new
Infiltrator 1500 gallon poly tank, a new D-box and 4 rows of 5, High Capacity Infiltrator
units , as shown on the plan.
