Dear Ms. DiRienzo and Members of the Conservation Commission:
BSC Group, Inc. (BSC) is pleased to submit this Request for an Certificate of Compliance (RCOC)
for DEP File #SE83-1621 forthe project authorized in the Order of Conditions (OOC) issued by the
Town of Yarmouth, DEP File #SE83-1621, for the property at 74 White Cedar Point Road (the Site).
The OOC was issued on February 12, 2003.
April 8, 2022, BSC conducted an inspection to confirm the current condition of the site. The
above -referenced Order of Conditions has lapsed and is therefore no longer valid, and the work
regulated by it was never started.

B. Submittal Requirements
Requests for Certificates of Compliance should be directed to the issuing authority that issued the final
Order of Conditions (OOC). If the project received an OOC from the Conservation Commission, submit
this request to that Commission. If the project was issued a Superseding Order of Conditions or was the
subject of an Adjudicatory Hearing Final Decision, submit this request to the appropriate DEP Regional
