RE: Subsurface Sewage Disposal System Inspection Report,27 Cleveland Way, West Yarmouth
Dear Mr. Putnam:
This department is in receipt of a subsurface sewage disposal system inspection report conducted
by John Graci on September 22,2000 and received in this office on October 4,2000.
The report identifies the septic system has having failed the inspection for the following reason:
1. The leach pits are full over the pipe and have no effective leaching left. The leach
pits are in hydraulic failure.
You were provided with a copy the failed inspection report by Shawn Horan. A copy of the letter
sent to Mr. Horan notifying him of the failure of the septic system is enclosed.
This office recently wrote to Mr.Horan asking him if he had any information regarding several of
the failed septic systems in the vicinity. A copy of the letter that was received from Mr. Horan is
enclosed. Mr. Horan advised that he had obtained attractive prices on engineering and
installation work and had notified the owners of this.
You have sixty(60)days from the date of this letter to obtain engineered plans and upgrade your
system to state Title 5 requirements.
Please contact me at the Health Department upon receipt of this letter so that we may discuss this
matter. I can be reached by telephoning the number printed above on Monday through Friday
from 9:00 to 11:00 AM during regularly scheduled office hours.
