Dear Mr. Murphy,
As per your request and in accordance with 310 CMR 15.021(3) of the State
Environmental Code-Title 5, an as-built review of the septic system at the subject site
has been conducted. I hereby certify that 1:he Sanitary Subsurface Disposal System
has been installed in substantial compliance with the design plan by Engineering
Works, Inc., dated 5/23/22. The septic system consists of a new 1500 gallon septic
tank, new distribution box and 5-LC6 precast chambers surrounded with 3.5' of stone,
as shown on the plan.
