Dear Mr. Murphy,
As per your request and in accordance with 310 CMR 15.021(3) of the State
Environmental Code-Title 5, an as-built review of the septic system at the subject site
has been conducted. I hereby certify that the Sanitary Subsurface Disposal System
has been installed in substantial compliance with the design plan by Engineering
Works, Inc., dated 5/27/22, revised 6/10/22. The septic system consists of a new 1500
gallon septic tank, new distribution box and 2-500 gallon precast H-20 vented chambers
surrounded with 4' of stone, as shown on the plan.
