In compliance with the Order of Conditions imposed on the above referenced property,
We hereby certify that the above mentioned property is in substantial compliance with
the Order of Conditions and request that a Certificate of Compliance be issued at your
earliest convenience.
The Order required all roof runoff to be controlled by gutters, downspouts and drywells.
During a site inspection on October 13, 2022, we noted eleven(11) downspouts, six(6)
were connected to drywells and five(5) were not flowing to drywells. The site, overall is
stable, with no observed erosion.
The Order of Conditions was recorded at the Barnstable Land Court Registry on June 27,
1984 as Doc. #3390360

B. Submittal Requirements
Requests for Certificates of Compliance should be directed to the issuing authority that issued the final
Order of Conditions (OOC). If the project received an OOC from the Conservation Commission, submit
this request to that Commission. If the project was issued a Superseding Order of Conditions or was the
subject of an Adjudicatory Hearing Final Decision, submit this request to the appropriate DEP Regional
