PROJECT NO: Z486.01 DATE: 10/17/22
LOCATION: 225 White’s Path, South Yarmouth, MA CLIENT: 225 White’s Path Units 2 and 3, LLC
WEATHER: Rain/Overcast, 52-59 ºF CONTRACTOR(S): DellbrookǀJKS; C.C.
Construction, Inc.
1. Mackenzie Meschwitz arrived on site at 0700 hours.
Left site at 1500 hours.
2. Earthwork equipment:
A) Caterpillar PC 360 LCi Excavator B) CASE SV216 Vibratory Roller
C) Bomag 50/55D Vibratory Plate Compactor
3. Earthwork observed:
A) CC Construction removed existing pavement from the proposed dock apron area along the westerly
side of the existing building (See attached Site Sketch, Figure No. 1 and Photo Log). The excavation
was advanced through about 2± inches of asphalt pavement and 2± inches into the underlying existing fill, which appeared to generally consist of brown to reddish-tan, fine to coarse SAND, some
fine to coarse Gravel, trace Silt. The excavation terminated in similar fill material. Excavation to
proposed subgrade elevations is scheduled to take place at a later date.
B) Drainage utility installation and backfilling took place today within the proposed pavement area located within the southwesterly portion of the site (see attached Site Sketch, Figure No. 1 and Photo
Log). C.C. Construction excavated a 3± foot deep trench, extending from about Elevations 38± to
35± feet for the installation of the HDPE drainage pipe and drainage structures. The excavation was advanced through apparent natural sand. The sand appeared to be generally representative of fill
sample FS-1 and appeared to generally consist of light tan, fine to medium SAND, trace (+) Silt, trace
Roots, and trace (-) fine Gravel. CC Construction placed crushed stone along the bottom of the trench
prior to placing the utilities.
C.C. Construction backfilled around the utilities with 12± inch thick lifts of the excavated sand. Each
lift was compacted with about 6± passes of the vibratory roller and/or vibratory plate compactor. The
lifts of backfill appeared firm and stable under the vibratory action of the roller and plate compactor.

The undersigned performed field density test nos. 5 through 8 on the compacted lifts of sand fill (see
attached Figure No. 1 for the approximate test locations). The test results indicated that a satisfactory
compaction percentage was achieved at each test location. Refer to the Summary of Field Density Tests sheet (page 3 of this report) for additional information.
C) Excavation for the installation of proposed utilities took place today within the proposed pavement
area located along the westerly side of the site (see attached Site Sketch, Figure No. 1). C.C.
Construction excavated about 2± feet of apparent similar natural sand from within the existing basin
area to accommodate installation of a drainage manhole and associated drainage lines. The excavation terminated at about Elevation 28± feet. The area is planned to be backfilled at a later date
following the utility installation.
