DAILY FIELD REPORT PROJECT: Proposed FedEx Distribution Facility FIELD REPORT NO: 6 PAGE 1 OF 3 PROJECT NO: Z486.01 DATE: 10/18/22 LOCATION: 225 White’s Path, South Yarmouth, MA CLIENT: 225 White’s Path Units 2 and 3, LLC WEATHER: Cloudy, 60-63 ºF CONTRACTOR(S): DellbrookǀJKS; C.C. Construction, Inc. 1. Mackenzie Meschwitz arrived on site at 0700 hours. Left site at 1500 hours. 2. Earthwork equipment: A) One Komatsu PC360LCi Excavator B) One Dynapac CA1400D Vibratory Drum Roller C) One Bomag 50/55D Vibratory Plate Compactor 3. Earthwork observed: A) C.C. Construction, Inc. installed drainage manhole structure DMH-5 and connecting 24-inch diameter HDPE drainage pipes at the proposed northwesterly parking area (see attached Site Sketch, Figure No. 1 and Photo Log). The manhole was placed over a layer of crushed stone in a previously excavated trench with the bottom of the structure at approximately Elevation (El) 28± feet. Trenches were excavated on the east and west sides of the manhole to facilitate installation of the connecting HDPE drainage pipes. The drainage pipes were connected to the manhole structure, then C.C. Construction backfilled around the manhole and pipes up to approximately El 38± feet with the previously excavated on-site sandy soil. The sandy backfill appeared to be generally representative of sample no. FS-1 and generally consisted of light tan, fine to medium SAND, trace Silt, trace fine Gravel, and trace (-) Roots. The backfill was placed in approximately 12± inch thick lifts. Each lift of backfill was compacted with about 8± passes of the vibratory drum roller and/or vibratory plate compactor. The lifts of backfill appeared firm and stable under the vibratory action of the roller and plate compactors. The undersigned performed field density test nos. 9 through 22 on the compacted lifts of backfill (see attached Figure No. 1 for the approximate test locations). The test results indicated that a satisfactory compaction percentage was achieved at each test location. Refer to the Summary of Field Density Tests sheet (page 3 of this report) for additional information.
