PROJECT NO: Z486.01 DATE: 10/27/22
LOCATION: 225 White’s Path, South Yarmouth, MA CLIENT: 225 White’s Path Units 2 and 3, LLC
WEATHER: Sunny, 58-62 ºF CONTRACTOR(S): DellbrookǀJKS; ADEP Group
1. Mackenzie Meschwitz arrived on site at 0700 hours.
Left site at 0900 hours. 2. Earthwork equipment:
A) One Wacker Neuson 1550 Vibratory Plate Compactor 3. Earthwork observed:
A) Per DellbrookǀJKS’ request, the undersigned visited the site to perform field density testing on previously placed and compacted utility backfill inside the building (see attached Site Sketch, Figure No. 1 for approximate locations and Photo Log). At the time of the undersigned’s site visit the noted areas had been backfilled up to about bottom of slab elevation. The exposed backfill material appeared to generally
consist of light tan, fine to medium SAND, trace fine to coarse Gravel, trace Silt. Delbrooks|JKS informed the undersigned that the previously excavated soil from the noted locations was used to backfill the trenches.
ADEP Group compacted the surface of the backfill at each location with an additional 6± passes with the vibratory plate compactor under the undersigned’s observation. The backfill appeared firm and stable under the vibratory action of the plate compactor.
The undersigned performed field density test (FDT) nos. 35 through 42 on the surface of the compacted
backfill in the noted areas (see attached Figure No. 1 for the approximate test locations). The test results indicated that a satisfactory compaction percentage was achieved at each test location. Refer to the Summary of Field Density Tests sheet on page 3 of this report for the test results.
