Construction, Inc. 1. Mackenzie Meschwitz arrived on site at 0700 hours. Left site at 1500 hours. 2. Earthwork equipment: A) One Volvo L120H Loader B) One CASE Construction SV216 Single Drum Roller C) One Caterpillar D6N Bulldozer 3. Earthwork observed: A) Placement of base course sand and gravel fill began within a proposed pavement area today (see attached Figure No. 1, Site Sketch and Photo Log). C.C. Construction placed one, 12± inch thick lift of base course sand and gravel fill in the southwesterly proposed pavement area. The base course fill, which was reportedly imported from C.C. Construction’s location in Plymouth, MA, appeared to be generally representative of sample no. FS-3. The imported fill appeared to generally consist of grayish brown, fine to coarse SAND and fine to coarse GRAVEL, trace Silt. The lift was placed over the previously compacted sandy subgrade up to approximately proposed bottom of pavement elevations. The lift of fill was compacted with about 8 passes of the vibratory single drum roller. The base course fill appeared firm and stable under the vibratory action of the roller. The undersigned performed Field Density Test (FDT) nos. 93 through 98 on the compacted base course sand and gravel fill (see attached Figure No. 1, Site Sketch for approximate test locations). The test results indicated that a satisfactory compaction percentage was achieved at each test location. Refer to the Summary of Field Density Tests sheet on page 3 of this report for additional information. 4.
