Subsurface Sewage Disposal System Installation Certification
Dear Mr. Murphy,
On October 18, 2022 the installation of the Subsurface Sewage Disposal System at the refer-
enced address was inspected. The system has been installed in substantial accordance with the
Subsurface Sewage Disposal Site Plan and Profile,produced by AJM Site Design, LLC dated
August 29, 2022 and approved by the Yarmouth Health Division. Please note there were origi-
nally 3 sewer outlet connections proposed on this plan. Only two connections were made as the
middle connection in the rear of the dwelling was replumbed and is no longer active. If you
