PROJECT NO: Z486.01 DATE: 11/28/22
LOCATION: 225 White’s Path, South Yarmouth, MA CLIENT: 225 White’s Path Units 2 and 3, LLC
WEATHER: Overcast, 38-43 ºF CONTRACTOR(S): DellbrookǀJKS; C.C.
Construction, Inc.
1. Mackenzie Meschwitz arrived on site at 0700 hours. Left site at 1500 hours.
2. Earthwork equipment:
A) One Volvo L120H Loader B) One Bomag 50/55D Reversible Vibratory Plate Compactor C) One Caterpillar 335F Excavator
D) One Dynapac CA1300D Single Drum Vibratory Roller 3. Earthwork observed:
A) Backfilling between the northerly ramp foundation walls along the exterior of the westerly building wall continued today (see attached Figure No. 1, Site Sketch and Photo Log). C.C. Construction placed two, 12± inch thick lifts of previously excavated sand in the northerly ramp area from approximately
Elevations 40± to 42± feet. The sand used as backfill appeared to be generally representative of sample no. FS-1 and generally consisted of tan, fine to medium SAND, trace Silt, and trace fine Gravel. Each lift of backfill was compacted with about six passes of the vibratory drum roller, followed by about five passes of the reversible vibratory plate compactor in the corners and along the walls. The lifts of backfill appeared firm and stable under the vibratory action of the compactors.
The undersigned performed field density test (FDT) nos. 137 through 142 on the compacted lifts of backfill (see attached Figure No. 1, Site Sketch for approximate test locations). The test results indicated that a satisfactory compaction percentage was achieved at each test location. Refer to the
Summary of Field Density Tests sheet on page 3 of this report for additional information. B) C.C. Construction began subgrade preparations within the proposed concrete dock apron area today by grading and proof compacting the exposed sand subgrade (see attached Figure No. 1, Site Sketch and Photo Log). The exposed sandy subgrade appeared to be generally representative of sample no. FS-1
and was observed to be similar in gradation as the sand backfill described in Item 3A. C.C. Construction proof-rolled the sandy subgrade with about five passes of the vibratory drum roller. The subgrade appeared firm and stable under the vibratory action of the roller.

The undersigned then performed FDT nos. 143 through 147 on the compacted sand subgrade (see attached Figure No. 1, Site Sketch for approximate test locations). The test results indicated that a satisfactory compaction percentage was achieved at each test location. Refer to the Summary of Field
Density Tests sheet on page 3 of this report for additional information. Delbrooks|JKS’ site superintendent confirmed that a minimum thickness of 12 inches of base course sand and gravel fill is planned to be placed within the proposed dock apron slab area.
