DAILY FIELD REPORT PROJECT: Proposed FedEx Distribution Facility FIELD REPORT NO: 21 PAGE 1 OF 3 PROJECT NO: Z486.01 DATE: 12/01/22 LOCATION: 225 White’s Path, South Yarmouth, MA CLIENT: 225 White’s Path Units 2 and 3, LLC WEATHER: Sunny, 36-47 ºF CONTRACTOR(S): DellbrookǀJKS; C.C. Construction, Inc. 1. Mackenzie Meschwitz arrived on site at 0700 hours. Left site at 1500 hours. 2. Earthwork equipment: A) One Volvo L120H Loader B) One Bomag 50/55D Reversible Vibratory Plate Compactor C) One Caterpillar 335F Excavator D) One Dynapac CA1300D Single Drum Vibratory Roller 3. Earthwork observed: A) C.C. Construction installed pre-cast concrete light pole bases around the perimeter of the proposed parking areas of the site (see attached Figure No. 1 – Site Sketch and Photo Log). Prior to the light pole base installations, C.C. Construction excavated a continuous 2± foot wide, 3± foot deep trench extending down from approximately Elevation (El) 40± feet at existing grade to about El 37± feet. The excavation was advanced into apparent natural sand generally consisting of tan to reddish tan, fine to medium SAND, trace Silt, trace Roots, and trace fine Gravel. A portion of the trench excavation at the southerly end of the site extended through about 1± foot of previously placed pavement base course sand and gravel fill prior to encountering the apparent natural sand. The base course gravel fill appeared to be generally representative of sample no. FS-3 and generally consisted of grayish brown, fine to coarse GRAVEL, some fine to coarse Sand, and trace Silt. The base of the trench excavation appeared firm and stable. The pre-cast concrete light pole bases were then placed at the bottom of the trench and held in place while the previously excavated sand was used as backfill around the bottom of the bases. After each concrete base was placed an electrical conduit was installed along the bottom of the trench. The previously excavated sand was used to backfill the majority of the trench back up to the existing ground surface at approximately El 40± feet. Within the portion of the trench which had been excavated through pavement base course fill, natural sand was placed as backfill up to approximately El 39± feet, followed by the previously excavated pavement base course fill up to El 40± feet (see attached Figure No. 1 – Site Sketch for approximate location). DAILY FIELD REPORT PROJECT: Proposed FedEx Distribution Facility FIELD REPORT NO: 21 PAGE 2 OF 3 PROJECT NO: Z486.01 DATE: 12/01/22 The backfill materials were placed in approximately 12± inch thick lifts. Each lift was compacted with about 4 passes of the vibratory roller or plate compactor and appeared firm and stable under the vibratory action of the compactors. The undersigned performed Field Density Test (FDT) nos. 148 through 159 on the compacted sand backfill and pavement base course fill (see attached Figure No. 1 – Site Sketch for the approximate test locations). The test results indicated that a satisfactory compaction percentage was achieved at each test location. Refer to the Summary of Field Density Tests sheet on page 3 of this report for additional information. 4. Attachments: Photo Log Figure No. 1 – Site Sketch 5.
