I, David D. Flaherty Jr., Registered Sanitarian, duly licensed in the Commonwealth of
Massachusetts, do hereby certify that I have visually inspected the system installation and certify
that the system, as constructed, was installed within substantial compliance and generally
conforms within acceptable tolerance to the regulations set forth in 310 CMR 15.000 and the
Town of Yarmouth Board of Health regulations. The following inspections were performed:
1) Bottom of the hole and Soil Removal for SAS-Inspection- 12/12/2022
2) Final Inspection- 12/13/2022
Such certification shall neither be misconstrued as a guarantee that the system will operate
satisfactorily nor certification of alterations after inspection.
