Dear Mr. Murphy:
Coastal Engineering Company, Inc. inspected the grease trap installation at the above referenced property
and determined that the grease trap had been installed in substantial conformance with the approved
design plan. The waste and vent pipes for the grease trap were installed by the plumbing contractor as
required by 248 CMR 10.00 Uniform State Plumbing Code and the Grease Trap Advisory dated October 24,
2011.
On behalf of the Cape Cod Collaborative, we hereby request the Yarmouth Health Department to issue a
Disposal Works Construction Permit Certificate of Compliance for the grease trap installation.
