In July 1997, the Commission issued an Order of Conditions for the above -referenced site, for construction of a single-family dwelling. More recently in August of 2020 the Commission issued an Order of Conditions for renovations to the existing dwelling. A site visit was made and a survey of the existing conditions relative to the Order was carried out. An as -built site plan and photos are enclosed for review. This is to certify, to the best of my knowledge and belief, that the work was performed in substantial compliance with the approved plan and Order of Conditions.
