STATE HIGHWAY LAYOUT PAVEMENT NOTES:
• PROPOSED ROADWAY (WITHIN SHLO) SHALL HAVE AN 8" GRAVEL BASE THAT HAS BEEN MACHINE
COMPACTED THEN PAVED WITH 4" OF HOT MIX ASPHALT (HMA) LAID IN TWO COURSES CONSISTING
OF A 2-1/2" BINDER COURSE AND A 1-1/2" FINISH COURSE.
• ANY EXISTING HMA BERM THAT EXISTS WITHIN THE PROPOSED DRIVEWAY MUST BE SAWCUT AND
REMOVED. THE NEW PAVEMENT MUST TIE INTO, AND NOT OVERLAP, THE EDGE OF THE HARDENED
SURFACE OF THE ROADWAY. ALL ABUTTING EDGES OF THE EXISTING PAVEMENT SHALL BE COATED
WITH A HOT POURED JOINT SEALER IMMEDIATELY PRIOR TO THE PLACEMENT OF THE HMA.

STORMWATER POLLUTION PREVENTION PLAN:
PRIOR TO CONSTRUCTION, A STORMWATER POLLUTION PREVENTION PLAN (SWPPP) WILL BE PREPARED AND
SUBMITTED ALONG WITH THE ACCOMPANYING NOTICE OF INTENT TO THE UNITED STATES ENVIRONMENTAL
PROTECTION AGENCY. THE SWPPP WILL CONTAIN ALL SPECIFICATIONS AND REQUIREMENTS FOR STORMWATER AND
EROSION CONTROL PROTOCOL DURING THE CONSTRUCTION PHASE. CONTRACTOR SHALL REVIEW THE SWPPP PLAN IN
DETAIL IN ADDITION TO THESE PLANS AND DETAILS PRIOR TO START OF CONSTRUCTION.

DRAINAGE CONSTRUCTION NOTES:
1.) ALL COMPONENTS, SUBJECT TO WHEEL LOADS, SHALL BE RATED H-20.
2.) ALL WORKMANSHIP PROVIDED SHALL BE IN CONFORMANCE WITH THE TOWN OF YARMOUTH
PLANNING BOARD & ENGINEERING SPECIFICATIONS.
3.) ANY MODIFICATIONS AND/OR DEVIATIONS FROM THE PLANS SHALL BE REVIEWED AND
APPROVED BY THE ENGINEERING DEPARTMENT.
CATCH BASIN AND LEACHING NOTES:
1.) ALL PROPOSED CATCH BASINS, LEACHING GALLERIES, AND TRENCHING ACROSS PARKING AREAS AND
SIDEWALKS SHALL BE BACKFILLED, AND COMPACTED IN 6-INCH LIFTS. COMPACTION SHALL MEET 95%
STANDARD PROCTOR REQUIREMENTS. ALL BACKFILL MATERIAL SHALL BE DENSE GRADED CRUSHED
STONE WITH A MAX SIZE OF 3/4" OR ENGINEERED APPROVED EQUAL.
2.) THE LEACHING CHAMBERS SHALL BE SET FULLY INTO CLEAN SAND. DESIGN ENGINEER SHALL INSPECT
THE SOILS AROUND AND BELOW THE LEACHING FACILITY PRIOR TO FACILITY INSTALLATION.

3.) ALL CAST IRON FRAMES AND SEALS FOR DRAINAGE COMPONENTS SHALL BE FINISHED WITH MORTAR

4.) CATCH BASIN GRATES SHALL BE PLACED OVER FILTER FABRIC UNTIL SITE IS PAVED WITH BINDER
COARSE. CONTRACTOR IS RESPONSIBLE TO MAINTAIN THE FILTER FABRIC SO SEDIMENT DOES NOT GET
INTO CATCH BASINS.
5.) EXISTING CATCH BASIN ON ROUTE 6A SHOULD BE FITTED WITH SILT SOCK SO AS TO PROTECT
DRAINAGE DURING CONSTRUCTION OF THE ROADWAY.
6.) THE TWO CATCH BASINS AT STA 1+95.14 ON FAITH ROAD SHALL BE FITTED WITH CASCADE STYLE
GRATES, IN ORDER TO HANDLE FAST-MOVING RUNOFF IN THE 6%SLOPE AREA OF THE ROAD.
