As the designer of the system for the referenced site, it was determined upon installation that the
system was installed per the requirements and allowances of the State Title V and the Yarmouth
Board of Health Regulations. All risers, tees, tee filters and operational alarm where observed at
the time of inspection. The issuance of this certification shall not be construed as a guarantee
that the system will function as designed.
