Dear Mr. Murphy,
As per your request and in accordance with 310 CMR 15.021(3) of the State
Environmental Code-Title 5, an as-built review of the septic system at the subject site
has been conducted. I hereby certify that the Sanitary Subsurface Disposal System
has been installed in substantial compliance with the design plan by Engineering
Works, Inc., dated 9/3/22. The septic system consists of a new 1500/500 gallon septic
tank/pump chamber, new distribution box and 3 rows of 3 Cultec C-4 Field Drain units,
as shown on the plan.
