Dear Mr. Murphy,
As per your request and in accordance with 310 CMR 15.021(3) of the State
Environmental Code-Title 5. an as-built review of the septic system at the subject site
has been conducted. I hereby certify that the Sanitary Subsurface Disposal System
has been installed in substantial compliance with the design plan by Sweetser
Engineering, P.O. Box 713, S. Dennis, MA 02660, dated 7/3/19, Revised 1/24/2020.
The septic system consists of a new 1500 gallon septic tank, distribution box and 4-
High Capacity Infiltrator chambers surrounded with stone. forming a 10' x 36' soil
absorption system. Sewer was sleeved for 10' each side of water service crossing..
