I certify that I understand the following requirements regarding any changes that may
be required for this project:
In accordance with paragraph 2(a) of section 1.03(General Procedures) of the OKH 972
CMR Rules and Regulations: Only minor changes may be approved by the Committee
without the filing of a new application and a new hearing. Minor changes include
alterations that can be done without a detrimental impact on the overall appearance of
the project such as altering a single window or door change or a minor change of
colors. All minor changes by amendment will require the local Committee's or its
designee's approval.
All changes to previously OKH approved plans require notification to and approval
from the local OKH Committee. Change requests must be submitted to the Committee
in writing on the appropriate request form, which may be obtained from the OKH office.
All change approvals must be obtained before incorporating the change into the
project.
If the change has been implemented prior to receipt of OKH approval, a Minor Change
approval or Certificate of Appropriateness application for the revised plans is still
required and will result in a doubled filing fee for the appropriate category of work.
Failure to comply with the above statements will result in the Building Department
issuing a stop -work order or delaying issuance of an Occupancy Permit or final
inspection approval.

We propose to install a solar photovoltaic (PV) system at the above listed address to reduce annual
electrical costs by generating onsite electricity.
Sumvind recommends installing the system} detailed below to optimise solar energy production:
Scope of Work
• Install a 10.5 kW (10,500 Watt) photovoltaic system using (28) LG 375 Watt or equal
• Install Enphase IQ7A microinverters to convert solar panel DC power into AC.
• The system shall be installed on black Snap N' Rack aluminum racking,
• Install components for data monitoring via the existing interact connection.
• SMART meter socket and AC disconnect to be located on the exterior per utility com]
• Install solar warning placards per code.

Net Metering: Massachusetts laws allow for all solar ener&• facilities to be interconnected to the
electrical service provider. The excess energy produced by a net -metered facility is exported to
the electrical grid and system owners are credited for all energy exported at a value roughly equal
to the prevailing price per kWh.
SMART Program: The state of Massachusetts SMART (Solar Massachusetts Renewable Target)
program is designed to help customers reduce their electrical energy costs with an added incentive.
The SMART program pays you monthly via check for the power generated by your solar system at a
set rate per kWh produced, for a term of 10 years.

Turnkey System:
-Conduct a site inspection, complete a written proposal and design drawings.
-Prepare HDC application and present to the board for approval of the project if required.
*Provide stamped engineered plans from MA licensed structural and electrical engineers as needed.
•Apply for building and electrical permits with the town inspectional services office.
-Submit the interconnection agreement with the utility- company.
-Submit the SMART application with the MA Department of Energy Resources (D.O.E.R.).
-Upon issuance of the building permit we shall expedite materials to the site.
-Install and commission the system.
*Schedule building and electrical inspections.
*Submit close-out documents to the utility company for approval to install SMART meter.
-Upon receipt of "permission to operate" from utility company we shall submit close-out documents to
D.O.E.R for final approval.
-Familiarize client with system monitoring and maintenance.
Installation:
-All work will be completed in accordance with Local, State and Federal building codes and accepted solar
installation practices. All work will be performed by licensed and/or properly trained personnel and shall be
performed in a manner consistent with the safety standards as required by OSHA and the Massachusetts
Department of Industrial Accidents.
Warranty:
•Sunwind, LLC shall warranty the installation (all parts and labor) of the solar system for 5 years from
the date of final inspection.
-The solar modules carry a manufacturer product & production warrant- of 25 years.
-The inverter manufacturer provides a 25-year warranty.
•Sunwind's labor warranty only covers equipment internet connectivity issues for 60 days after the date of
final inspection. Service calls beyond this range will be charged an hourly service rate.
General Notes:
•$unwind, LLC shall make every effort to provide clients with the most up to date incentive information.
•Sunwind. LLC shall not be held liable for any change in the amount of incentives or ener8y attributes
estimated to be received.
-We encourage all clients to discuss the incentives with their accountant to determine current and future
