Dear Mr. Murphy,
As per your request and in accordance with 310 CMR 15.021(3) of the State
Environmental Code-Title 5, an as-built review of the septic system at the subject site
has been conducted. I hereby certify that the Sanitary Subsurface Disposal System
has been installed in substantial compliance with the design plan by Engineering
Works, Inc., dated 5/10/22. The septic system consists of a new 1500 gallon plastic
septic tank, new distribution box and 3 rows of 3 Cultec CA Field Drain units, as shown
on the plan. Water service was relocated per plan.
