On May 16, 1991, the Commission issued an Order of Conditions for construction of a wood bulkhead, dredging and to permit a platform with ramp and floats within a man-made lagoon. Extensions were granted, with the final expiration in 1999. A site visit was made for the purpose of closing out the Order of Conditions and to perform an as -built. The bulkhead was located and found to be installed as proposed. Dredging was performed in the 90's according to the owner's relative, but the lagoon has since filled in. No platform or ramp with floats were installed. This is to certify, to the best of my knowledge and belief, that the work was performed in substantial compliance with the approved plan and Order of Conditions, with the exceptions of the above. We hereby request the issuance of a Certificate of Compliance at this time. Thank you for your consideration. Very truly yours, Daniel A.
