On May 16, 2017 the Commission issued an Amended Order of
Conditions for the above -referenced site, for a raze and replace
with proposed pool/patio. The record (revised) plan date
referenced by this Amended Order was 4/20/2017. Subsequently,
minor revisions to incorporate adjustments to the pool, patio and
mitigation were reflected in a final revised plan dated October
26, 2017 which is on file with the town.
Chris Kenney, the building contractor, indicated that Kelly Grant
(the Conservation Agent at the time) performed a detailed and
thorough walk-through with him late spring of 2018 prior to
issuance of the Occupancy Permit, referencing the plans and
paperwork involved with the site. Her only issue at that time
was that 3 blueberry bushes didn't look healthy and needed
replacing (which was done). Down Cape Engineering performed an
as -built of the mitigation areas, dwelling and pool/patio and
found that the mitigation area needed to be increased. The
builder took up lawn and added the necessary plantings square
footage. Down Cape re -measured and found that the area conformed
with that which was required, with minor location differences.
The as -built is enclosed.
Drywells were installed for roof run-off and a drywell it was
installed for pool drawdown.
To the best of my knowledge and belief, the site substantially
complies with the Order of Conditions and the plan of record, as
well as the revised site plan dated October 26, 2017.
We hereby request the issuance of a Certificate of Compliance.
