The project consists of installing a patio, plunge pool with associated pool equipment and fencing within land subject to coastal storm flowage. This work is located on the same property where the applicant recently completed construction of a single-family residence in accordance with Order of Conditions issued by the Commission (File #083-2309). The filing deadline is two weeks prior to the next scheduled Conservation Commission meeting for new applications. Failure to follow this Checklist shall result in an Administratively Incomplete Application and will not be advertised for a Public Hearing. Refer to the meeting schedule. Contact the Conservation Office if you need assistance. Submitting an Application: Hardcopy filing dropped off or mailed to the Conservation Office including: RDA Administrative Checklist Complete RDA application with original signatures (double sided). * Detailed narrative of the project with sufficient information to enable the Commission to determine whether the proposed work will alter an area subject to protection. BVW delineation sheet(s), coastal bank calculation and other relevant material if applicable (not applicable for flood zone only projects). Yarmouth Assessor’s locator map highlighting parcel(s) where work is proposed, including map and parcel numbers 1 original and 7 photocopies of the plan/sketch, folded separately, right side out with title and address visible. All plans shall reference NAVD1988 unless otherwise noted. Landscape plans shall be detailed to show proposed and existing conditions, native species, size and spacing. Initial below I CERTIFY that all on-site requirements will be completed by noon on the Friday prior to the hearing date. All proposed structures must be staked, and all relevant resource areas and buffer zones must be staked or flagged. Please consult the Yarmouth Wetland Regulations, page 17. * If the applicant is NOT the property owner: Please submit a signed Site Access Authorization Form; AND The owner must be sent a copy of the application via Certified Mail on the same day it is filed with this office. This work is located on the same property where the applicant recently completed construction of a single-family residence in accordance with Order of Conditions issued by the Commission (File #083-2309). The proposed plunge pool is 7’ x 13’ with a maximum depth of 5’ and total capacity of 3,400 gallons. The proposed patio is 12’ x 16’. These features along with pool equipment and fencing are depicted on the Plot Plan for Plunge Pool submitted with the application. Typical equipment involved in the construction of the plunge pool will be a mini-excavator and/or skid steer. Staging areas will be in the rear yard. This project is not subject to the standards of the MA stormwater standards as it is a single-family home. Any plantings will consist of native species and invasive species will be removed if identified. This project will not reduce the land subject to coastal storm flowage as the volume is negligible to the volume of storm flowage. Stormwater paths will remain unchanged. No significant change to the surface topography is proposed.
