On November 16, 2005, Roberts Septic Service was issued a permit to install a Title V Septic System at
i 4-0 i_t_ane, Yarmouth, MA, based on a design drawn by Shay Environmental Services;
Inc.
I Certify That The Septic System Referenced Was Installed Substantially According to the Plan
XX I Certify That the Referenced Above Septic System Was Installed With Changes but in
Accordance With State and Local Regulations, Revisions or As-Built Plans/Sketch will Follow.
1. Tank moved 2' toward left lot line to avoid deck support.
�. D-Box placed on other end of SAS.
3, Verified 4 feet of pervious material beneath SAS on 11/26/05 Elevation of bottom of SAS =
94.48_ C-2 Layer found at elevation 90.20.
The Septic System Was Not Installed Per State and Local Regulations and Corrective Action is
Required.
